About the Summit

Australia's hospital, disability, aged care, mental health and community care systems face growing challenges in supporting people with complex needs to transition safely from hospital into appropriate care and accommodation.

Delayed hospital discharge, NDIS funding periods, access to allied health assessments, shortages in suitable supported accommodation, difficulties securing in-home supports, aged care service availability and workforce capacity continue to create significant barriers across these interconnected systems.

The 2027 National Integrated Care, Complex Needs & Hospital-to-Home Summit will bring together government representatives, hospital executives, health professionals, allied health specialists, NDIS and disability organisations, aged care leaders, mental health professionals, housing providers, researchers and technology innovators.

The 2-day Summit will examine the clinical, operational, funding and accommodation challenges affecting hospital-to-community transitions, while exploring innovative models of care, emerging research, policy developments and practical solutions.

Importantly, the Summit will consider the entire continuum of care — from hospital discharge and rehabilitation through to independent living, in-home support, Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), mental health accommodation, aged care and long-term community-based care.

The focus is not simply on getting people out of hospital, but ensuring the right funding, clinical care, workforce, accommodation and ongoing supports are available to enable safe and sustainable transitions.

Key Topics & Areas of Discussion
