Session Overview
Transitions from hospital to home are complex for participants with high support needs, mental health conditions, or multiple disabilities.
The biggest challenge in this space is bridging the gap between the health system and housing/disability providers, ensuring safe, sustainable, and person-centred outcomes.
This Masterclass session provides hospital discharge planners, NDIS engagement leads, behaviour practitioners, occupational therapists, and allied health professionals with practical tools, frameworks, and collaborative strategies to navigate these transitions effectively.
Attendees will learn how to align clinical evidence, functional assessments, and NDIS SDA/SIL pathways to secure timely housing, reduce discharge delays, and improve participant outcomes.
