Event Overview

Delays in transitioning NDIS participants from hospital into appropriate community-based accommodation and supports continue to present significant challenges across Australia's health and disability sectors.

NDIS funding periods, delays in plan reviews and funding decisions, difficulties securing suitable accommodation, workforce availability, assistive technology requirements and communication barriers between organisations can all contribute to extended hospital stays, complex discharge arrangements and uncertainty for participants, families and professionals.

The Perth National Hospital-to-Home CarePathways Forum & Roundtable brings together hospital and health professionals, NDIS representatives, Support Coordinators, allied health professionals, disability service providers, Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) organisations and other community-based services to examine the practical, operational and governance challenges affecting hospital-to-community transitions.

This professional forum combines practical information sessions with facilitated cross-sector discussions, providing an opportunity to better understand the responsibilities, limitations and operational pressures experienced across the hospital, NDIS, housing and disability support systems.

A central focus will be the impact of NDIS funding periods, funding availability and delays in implementing approved supports on hospital discharge planning, particularly where funding arrangements do not align with the timing or level of support required for a participant to transition safely into the community.

Discussions will also examine the distinction between being clinically ready for discharge and having appropriate community arrangements in place, including accommodation, equipment, workforce capacity, functional assessments, support coordination and continuity of care.

The forum recognises that successful hospital-to-home transitions require more than discharge approval. They depend on appropriate planning, timely information sharing, clearly understood responsibilities and coordinated arrangements across multiple organisations operating within different legislative, funding and governance frameworks.

Key Topics & Areas of Discussion

NDIS Funding Periods, Funding Delays & Hospital Discharge

Examining how NDIS funding arrangements can affect discharge planning and the establishment of community-based supports.

Discussions will include funding periods, plan reviews, changes in circumstances, funding availability, delays in implementing approved supports and circumstances where a participant's funded arrangements may not align with their current needs or discharge requirements.

Roles, Responsibilities & Hospital-to-Home Pathways

Clarifying the respective roles of hospitals, the NDIA, NDIS Hospital Liaison Officers, Support Coordinators, allied health professionals, disability providers and accommodation organisations.

This session will examine when different organisations become involved, the distinction between health and disability responsibilities, and how communication, coordination and escalation can support complex transitions.

NDIS Home and Living Arrangements – SIL, SDA & Community Supports

A practical discussion of the NDIS home and living arrangements relevant to participants transitioning from hospital, including Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Individualised Living Options (ILO) and Medium Term Accommodation (MTA).

Consideration will be given to accommodation availability, support requirements, eligibility, funding arrangements and the practical challenges associated with establishing appropriate community-based living environments.

From Clinical Readiness to Community Readiness

Exploring the difference between a participant being medically ready for discharge and the community being prepared to receive them safely.

Key considerations include functional support requirements, medication continuity, allied health involvement, assistive technology, equipment, home modifications, workforce readiness, accommodation arrangements and the transfer of essential information to receiving services.

Communication, Consent & Information Sharing

Examining the communication barriers that can arise between hospital teams, Support Coordinators, participants, families, nominees and community-based providers.

Discussions will consider participant consent, privacy, access to relevant NDIS plan and funding information, timely communication, professional boundaries and the importance of accurate information throughout the transition process.

Clinical Governance, Legal Responsibilities & Transition Risk

A professional discussion examining the clinical, legal and governance considerations associated with complex hospital-to-community transitions.

Areas of focus include safe and timely discharge planning, professional accountability, documentation, decision-making, participant choice and consent, continuity of care and the management of identified risks where community arrangements remain incomplete.

Hospital-to-Home Cross-Sector Roundtable

Supporting Sustainable Transitions & Reducing Avoidable Returns to Hospital

An important component of the Perth forum is the facilitated cross-sector roundtable, bringing together hospital and health professionals, NDIS representatives, Support Coordinators, allied health professionals, disability providers and community-based services.

The roundtable provides an opportunity for attendees to share their experiences, identify recurring barriers and gain a greater understanding of the challenges encountered by different organisations throughout the hospital-to-home pathway.

Hospital and health representatives will be invited to discuss the factors contributing to extended hospital stays, challenges involving NDIS funding and support availability, accommodation delays, communication with external organisations and the operational pressures associated with complex discharge planning.

Disability and community-based representatives will have the opportunity to discuss the challenges involved in establishing funded supports, securing appropriate accommodation, preparing frontline workforces, accessing equipment and receiving the clinical and functional information necessary to support participants following discharge.

A particular focus will be understanding why some NDIS participants remain in hospital despite being clinically ready for discharge, and why others experience an unplanned or potentially avoidable return to hospital after transitioning into the community.

The discussion will consider opportunities for earlier identification of barriers, improved coordination, clearer professional responsibilities and greater continuity between hospital and community-based services.

The forum is designed to encourage constructive, informed discussion and practical information sharing while respecting the distinct responsibilities, legislative requirements and professional boundaries of participating organisations.

Who should attend?

This forum is relevant to professionals and organisations involved in hospital discharge planning, disability support, complex care and community-based transitions, including:

Supporting Better Hospital-to-Home Outcomes

The objective is not simply to achieve hospital discharge, but to support safe, coordinated and sustainable transitions into the community.

The Perth National Hospital-to-Home CarePathways Forum & Roundtable provides a valuable opportunity for professionals across the health and disability sectors to better understand the challenges affecting hospital discharge, NDIS funding, accommodation, community support readiness and continuity of care.

Through practical information sharing and facilitated cross-sector discussion, the forum aims to strengthen professional understanding, clarify responsibilities and examine opportunities to reduce unnecessary hospital delays and avoidable readmissions.

Places are limited, and early registration is encouraged.

Please note: The published program is provided as a guide and may be adjusted to reflect emerging issues, attendee contributions and the direction of the facilitated roundtable discussions.

NDISDA® & Health2Ageducate®

Hospital to Home

IMPORTANT INFORMATION, PROFESSIONAL ENGAGEMENT REQUIREMENTS, COLLABORATION & DISCLAIMER

1. About the Hospital to Home Program

The NDISDA® & Health2Ageducate Hospital to Home Queensland program brings together two specialist professional networks to support education, cross-sector dialogue, professional collaboration and stronger understanding of hospital-to-community transition pathways.

NDISDA® SDA Housing & Disability supports the disability and NDIS component of Hospital to Home, including professional engagement across disability supports, Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Support Coordination, allied health, housing and other relevant community-based pathways.

Health2Ageducate® supports the health and aged care component, including professional engagement across aged care, Support at Home, Transition Care, restorative care, rehabilitation, allied health, in-home support and other relevant services for older people transitioning from hospital into the community.

Neither NDISDA® nor Health2Ageducate provides direct participant or client care, clinical care, Support Coordination, SIL, SDA, aged care or direct support services.

The Hospital to Home program operates as an independent professional education, engagement and collaboration initiative, bringing together relevant stakeholders to strengthen professional understanding, identify system challenges and encourage constructive solutions.

2. Purpose of Hospital to Home Roundtable Engagements

Hospital to Home forums and roundtables are professional education, information-sharing and cross-sector collaboration initiatives. They are not commercial solicitation, referral-generation or provider marketing events.

These engagements bring together hospital and health representatives, disability and NDIS-sector professionals, aged care and community-based services, allied health and rehabilitation professionals, researchers, industry experts, government representatives and other relevant stakeholders.

The purpose of these roundtables is to:

  • Provide a safe, respectful and professionally facilitated environment for hospital personnel and other invited representatives.

  • Encourage open and constructive discussion about hospital discharge challenges, transition pathways, service coordination and systemic barriers.

  • Strengthen understanding of the respective responsibilities of hospitals, disability services, aged care, allied health, government agencies and community-based organisations.

  • Identify opportunities for improved communication, professional education and cross-sector collaboration.

  • Explore practical solutions to recurring challenges affecting hospital-to-community transitions.

  • Identify systemic issues that may warrant further engagement with relevant government agencies, regulators, decision-makers and policymakers.

  • Promote ethical, appropriate and person-centred approaches to hospital-to-community transitions.

Hospital personnel are invited to share their professional experiences, observations and challenges without any expectation or obligation to provide referrals, arrange provider introductions, disclose patient information or enter into commercial discussions.

The primary purpose of these engagements is to listen, learn, collaborate and identify constructive solutions—not to facilitate commercial access to hospitals or patients.

3. NDISDA® Membership, Code of Conduct and Participation Requirements

The Hospital to Home forums and roundtable engagements are generally restricted to NDISDA® Members, who are required to abide by the NDISDA® Member Code of Conduct and maintain appropriate standards of professional behaviour, ethical engagement, accountability and respect for participating organisations.

This membership-based approach supports a professionally governed environment in which hospitals and other invited representatives can participate with confidence.

Non-members may occasionally be invited to participate at the discretion of NDISDA®, where their professional expertise, experience, organisational role or contribution is considered relevant to the purpose of the engagement.

All invited non-members are equally expected to comply with the professional conduct, confidentiality, privacy and non-solicitation requirements of the forum.

Hospital personnel, government representatives and other invited professional contributors are not required to hold NDISDA® membership unless expressly specified for a particular Event.

Participation in a Hospital to Home forum does not provide any member or non-member with preferred-provider status, referral privileges, hospital endorsement or guaranteed access to patients, participants, service opportunities or commercial arrangements.

NDISDA® reserves the right to determine attendance eligibility, restrict invitations or decline participation where reasonably necessary to protect the professional purpose, independence and integrity of the engagement.

4. Strict Non-Solicitation and Protection of Hospital Personnel

Hospital to Home is not a provider solicitation, referral-seeking, marketing or business development event.

NDISDA® and Health2Ageducate are committed to ensuring that hospital personnel, discharge planners, social workers, NDIS liaison officers, patient flow teams, allied health professionals, government representatives and other invited professionals feel safe, respected and comfortable participating in these roundtable engagements.

Hospital representatives must be able to speak openly about operational challenges, discharge barriers and systemic issues without being approached, pressured, harassed or targeted by providers seeking referrals, placements, patient access or commercial opportunities.

Accordingly, participating members, invited non-members, providers, speakers, sponsors and other attendees must not:

  • Approach hospital personnel for the purpose of obtaining patient referrals, placements or preferred-provider arrangements.

  • Pressure hospital staff to arrange meetings, introductions or commercial engagements.

  • Request access to patient lists, discharge information, individual cases or confidential hospital information.

  • Engage in unsolicited marketing, persistent follow-up, aggressive networking or other conduct that may make hospital personnel uncomfortable.

Use information shared during a roundtable to identify or target patients, families or individuals requiring services.

Represent or imply that attendance at a Hospital to Home forum provides preferred-provider status, hospital endorsement or guaranteed access to referrals.

Contact hospital personnel after an Event for unsolicited commercial purposes solely because they participated in the roundtable.

Use the roundtable or its attendee information for unauthorised marketing, referral-seeking or commercial approaches.

All requests for provider introductions, professional engagement, training or potential service connections arising from the roundtable should be directed through NDISDA® or Health2Ageducate, as appropriate.

Any subsequent introduction or engagement will be considered on its merits, subject to the relevant organisation's consent, governance requirements, procurement processes and professional responsibilities.

There is no guarantee that an introduction, meeting, referral or commercial engagement will be facilitated.

NDISDA® reserves the right to intervene, restrict future participation or remove attendees who breach these professional engagement requirements, subject to applicable law.

5. Invitation to Collaborate – Strengthening Hospital-to-Community Care Pathways

NDISDA® and Health2Ageducate actively welcome and encourage genuine professional collaboration between hospitals, health services, government agencies, disability and aged-care providers, allied health professionals, community organisations, researchers and other relevant stakeholders.

Our Hospital to Home roundtable engagements are designed not only to identify challenges but also to encourage constructive dialogue, shared learning and practical opportunities for collaboration that improve hospital-to-community transitions.

We welcome collaboration in areas including:

  • Professional education and information sessions: Addressing identified hospital discharge and transition challenges, including NDIS processes, disability and aged-care pathways, provider responsibilities, funding arrangements and relevant compliance requirements.

  • Cross-sector engagement: Improving communication, understanding and coordination between hospitals, disability services, aged care, allied health and community-based organisations.

  • Joint initiatives and working groups: Exploring practical approaches to recurring discharge barriers, service gaps and complex transition pathways.

  • Research, policy and systemic improvement: Identifying opportunities to engage with government agencies, regulators, policymakers and decision-makers on recurring system challenges.

  • Service pathway awareness: Supporting hospital personnel to better understand available disability, aged care, rehabilitation, housing and community support options.

  • Future roundtables and forums: Bringing together hospital representatives, industry professionals, government agencies and relevant policy and decision-makers to continue discussions and review progress.

  • Collaborative education and training: Developing tailored information and training sessions in response to challenges identified by participating hospitals and health services.

Collaboration is encouraged. Unsolicited solicitation is not.

We welcome genuine professional relationships, knowledge sharing, appropriate introductions and collaborative initiatives. However, these must never involve pressure on hospital personnel for referrals, patient access, provider appointments or commercial opportunities.

Hospital personnel must feel safe and comfortable participating in open discussions without being subjected to unsolicited marketing, persistent approaches or requests for referrals.

Organisations interested in collaborating, contributing expertise, delivering information sessions or exploring joint initiatives are invited to contact NDISDA® or Health2Ageducate directly.

All proposed collaborations will be considered in accordance with the professional purpose of the program, relevant governance requirements, privacy obligations and the independence of participating hospitals and organisations.

Our aim is to build trusted professional relationships, strengthen cross-sector cooperation and work collectively towards safer, more coordinated and sustainable hospital-to-community care pathways.

6. Education, Connection and Facilitated Service Pathways

Through Hospital to Home, NDISDA® and Health2Ageducate seek to strengthen professional understanding, communication and coordination across hospital-to-community transition pathways.

Where assistance is requested, either network may help identify relevant services, organisations or potential pathways through its respective professional network and, where appropriate, facilitate introductions to approved participating organisations.

Such assistance does not constitute a clinical recommendation, formal referral, provider endorsement, care coordination service or guarantee of suitability.

Any facilitated introduction is subject to:

  • The individual's choice and control;

  • Informed consent;

  • The individual's preferences, circumstances and support requirements;

  • The right to select, decline or change a provider or service;

  • Applicable privacy and confidentiality requirements;

  • Hospital and health-service governance and discharge processes;

  • Relevant procurement and conflict-of-interest requirements; and

  • Applicable professional, regulatory and statutory obligations.

The individual, or their authorised representative where applicable, retains responsibility for deciding which provider or service they wish to engage.

Hospital personnel and treating professionals retain their respective clinical, discharge-planning and professional responsibilities.

NDISDA® and Health2Ageducate do not guarantee that any particular provider, service or organisation will be available, appropriate, approved or selected.

7. Fees and Funding

NDISDA® and Health2Ageducate do not take, deduct or receive fees from an individual's NDIS participant plan, aged-care funding allocation, government-funded care package or other individual government-funded support plan.

Any separate professional education, consultancy, training, membership or event-related fees are distinct from an individual's funded care or support arrangements and will be disclosed where applicable.

Participation in Hospital to Home does not guarantee NDIS funding, aged-care funding, hospital referrals, service agreements, accommodation placements or other commercial outcomes.

8. Professional Conduct, Governance and Confidentiality

All participating organisations, members, invited non-members, speakers and attendees are expected to maintain appropriate professional standards throughout Hospital to Home engagements.

This includes respecting:

  • Patient, participant and client privacy and confidentiality;

  • Individual choice, dignity, independence and informed consent;

  • Hospital and health-service governance and operational procedures;

  • Government procurement and conflict-of-interest requirements;

  • Professional boundaries and organisational responsibilities;

  • Relevant legislation, regulatory obligations and codes of conduct; and

  • The purpose and integrity of the roundtable engagement.

  • Individual patient or participant cases should be discussed using de-identified information wherever practicable.

No attendee should disclose identifiable, sensitive or confidential information unless lawfully authorised and appropriate in the circumstances.

Professional conversations, relationship-building and the mutual exchange of contact details are welcome where the engagement is genuine, appropriate and welcomed by all parties involved.

The presence of hospital, government or statutory representatives at a Hospital to Home forum must never be interpreted as an endorsement of any participating provider, organisation or service.

9. Post-Transition Consultancy and Professional Follow-Up

Where a member has been engaged through an NDISDA® or Health2Ageducate-facilitated pathway, the relevant network may undertake structured professional consultancy follow-up at approximately three-month intervals, where appropriate and agreed.

The purpose of this follow-up is to review the participating member's professional engagement and identify service, communication, coordination or transition concerns that may require attention.

This process is intended to support professional accountability, good governance and ongoing improvement within facilitated pathways.

This is not participant or client monitoring, clinical oversight, care coordination, case management or direct service delivery.

Neither NDISDA® nor Health2Ageducate assumes the responsibilities of treating practitioners, Support Coordinators, aged-care providers, care managers, service providers, hospitals or other responsible parties.

Any follow-up will be undertaken subject to applicable consent, privacy, confidentiality and information-sharing requirements.

Where broader or recurring issues are identified, de-identified findings may inform future Hospital to Home education, professional forums, research and cross-sector engagement.

The intention is to contribute to continuous improvement across disability, aged care, health and community-based transition pathways.

10. Independent Professional Information and Disclaimer

Hospital to Home forums and roundtables are provided for professional education, information sharing and cross-sector discussion.

Statements, opinions, recommendations and information presented by speakers, panellists, facilitators, members and other attendees are those of the respective contributors unless expressly stated otherwise.

They do not necessarily represent the views or positions of NDISDA®, Health2Ageducate, participating hospitals, government agencies, statutory authorities or other organisations.

Information shared during the forums should not be relied upon as individual clinical, legal, financial, NDIS funding, aged-care, discharge-planning or other professional advice.

Neither NDISDA® nor Health2Ageducate makes clinical decisions, determines individual eligibility for funded services, approves discharge arrangements or guarantees the suitability, availability or performance of any independent service provider.

Participating organisations and professionals remain responsible for their own decisions, statutory obligations, professional conduct, service delivery and governance requirements.

11. Commitment to Safe, Respectful and Collaborative Roundtable Engagement

NDISDA® and Health2Ageducate recognise that meaningful hospital-to-community collaboration depends on trust, professional integrity and the willingness of participating organisations to share experiences openly.

The generally member-based nature of these engagements, supported by the NDISDA® Member Code of Conduct, is intended to provide an additional level of professional accountability and confidence for participating hospitals and other invited representatives.

Hospital personnel must feel confident that their participation in these engagements will not expose them to unsolicited provider approaches, pressure for referrals, harassment or inappropriate commercial expectations.

These roundtables are designed to listen, learn, identify challenges, share knowledge, encourage collaboration and explore constructive solutions—not to create a marketplace for patient referrals.

We welcome the involvement of hospitals, government agencies, disability and aged-care organisations, researchers, industry experts and other stakeholders who share our commitment to improving hospital-to-community transitions.

All participating members, invited non-members, speakers and attendees are expected to respect these principles as a condition of participation.

Our priority is to protect the integrity of these professional engagements, respect the responsibilities of hospital and health-service personnel, and encourage genuine collaboration that contributes to stronger, safer and more coordinated hospital-to-community care pathways.
