The NDISDA Hospital to Home Forums, Roundtable and Workshops form part of Australia's leading Hospital to Home Engagement Program, designed to strengthen communication between hospitals and the disability sector while improving workforce capability and participant outcomes.
Developed from years of national collaboration with the disability sector, government, hospitals, the NDIS Quality and Safeguards Commission and industry leaders, these practical workshops equip professionals with the knowledge, confidence and collaborative strategies required to support safe, timely and person-centred
hospital-to-home transitions.

Participation Eligibility
The NDISDA Hospital to Home Engagement Program has been established to bring together hospital professionals and the disability sector in a trusted, collaborative and professionally governed environment.
Participation is open to:

Hospital and Health Service personnel involved in discharge planning, patient flow, community transition and related clinical or leadership roles. Hospital personnel are not required to be NDISDA members.

NDISDA Platinum Members and NDISDA Strategic Growth Housing Partners from the disability sector.
Governance
To maintain the integrity of the program and uphold the highest standards of governance, participation by disability organisations is exclusively available to eligible NDISDA members.
This ensures participating organisations have committed to the NDISDA Code of Conduct, ethical practice, collaboration, professionalism, transparency and participant-centred service delivery.
This governance model provides hospitals with confidence that participating disability organisations have demonstrated a commitment to quality, accountability and collaborative practice, while preserving each participant's right to informed decision-making, choice and control.
Participation in the Hospital to Home Engagement Program does not constitute a referral arrangement.
The program has been developed to strengthen education, communication, collaboration and hospital-to-home pathways for people living with disability.
