Workshop Overview

Successful hospital-to-home transitions require far more than clinical readiness.

They depend on effective communication, clearly defined responsibilities, coordinated planning and a practical understanding of the disability sector, housing pathways and the National Disability Insurance Scheme (NDIS).

Developed as part of the NDISDA Hospital to Home Engagement Program, this practical and interactive workshop has been specifically designed for hospital professionals involved in discharge planning, patient flow and community transition.

Participants will gain a greater understanding of disability support pathways, the roles of hospitals and community providers, Specialist Disability Accommodation (SDA), Supported Independent Living (SIL), Support Coordination and the collaborative approaches required to support safe, timely and participant-centred hospital discharge.

Using real-world case studies and facilitated discussion, participants will examine common challenges encountered throughout the hospital-to-home journey while developing practical strategies to improve communication, strengthen multidisciplinary collaboration and reduce avoidable discharge delays.

Developed as part of the NDISDA Hospital to Home Engagement Program, this advanced full-day workshop provides hospital professionals with a comprehensive understanding of complex discharge planning and the practical strategies required to achieve safe, timely and sustainable transitions from hospital into the community.

Building on the foundations of the Hospital to Home Workshop, all attendees will explore complex case studies, disability housing pathways, governance considerations, participant choice and control, communication strategies and collaborative planning with community providers.

The workshop combines expert presentations, facilitated discussion, multidisciplinary activities and real-world case studies to strengthen capability across hospitals and improve participant outcomes.

A 3-Month Organisational Capability and Service Improvement Program

Improving hospital-to-home transitions requires more than a single education session.

Sustainable improvement is achieved by building workforce capability, strengthening communication across multidisciplinary teams and embedding consistent practices that support safe, timely and participant-centred discharge planning.

Developed as part of the NDISDA Hospital to Home Engagement Program, this 3-month program is designed for hospitals and health services seeking to strengthen organisational capability while improving collaboration with the disability sector.

The program combines NDISDA's 2 core education workshops with a facilitated service improvement workshop, allowing participants to progressively build their knowledge, apply learning within their workplace and return to review practical outcomes and opportunities for continuous improvement.

Program Structure
Month 1
Hospital to Home Foundations Workshop (Half Day – 4 Hours)
Participants complete the Hospital to Home Foundations Workshop, providing the essential knowledge required to understand hospital-to-home pathways, the NDIS, disability housing, Support Coordination, multidisciplinary communication and participant-centred discharge planning.

Month 2
Advanced Hospital to Home Practice Workshop (Full Day – 7 Hours)
Participants then undertake the Advanced Hospital to Home Practice Workshop, expanding their knowledge through complex case studies, advanced NDIS pathways, disability housing, governance, collaborative discharge planning and practical problem-solving.
Between workshops, participants are encouraged to apply their learning within their own workplace and identify opportunities to improve communication, coordination and discharge planning.

Month 3
Leadership, Service Improvement and Embedding Best Practice
Half-Day Facilitated Workshop (4 Hours)

Cost : $1500pp
The final workshop is designed to consolidate learning and help organisations translate education into sustainable practice.
Rather than introducing new theory, this highly interactive session focuses on reviewing implementation, identifying organisational challenges and developing practical strategies to strengthen hospital-to-home pathways within the participating hospital or health service.
Facilitated by NDISDA, participants work collaboratively to evaluate current practices, discuss real hospital experiences, identify ongoing barriers and develop practical actions that support continuous improvement.
